5. Occupational-Health Information

Occupational-health services may involve employers, government agencies, licensing organizations, laboratories, or other authorized parties.

Pro-Fit will disclose examination, testing, certification, or work-status information only when permitted by law, supported by appropriate authorization, or required for the requested service.

Employers do not automatically receive complete medical records.

6. Medical Records and Security

Pro-Fit uses reasonable administrative, technical, and physical safeguards intended to protect medical information from unauthorized access, loss, alteration, or disclosure.

No electronic system is completely risk-free. Highly sensitive information should be submitted only through an approved secure process and not through a general website form or ordinary email.

7. Complaints

You may submit a privacy complaint to Pro-Fit without fear of retaliation.

You may also submit a complaint to the appropriate government privacy authority when you believe your health-information privacy rights have been violated.

Pro-Fit will not retaliate against anyone for making a good-faith complaint.

8. Changes and Questions

Pro-Fit may revise this notice when its privacy practices or legal responsibilities change.

The revised notice may apply to information already maintained and will be made available through the website or service location.
